Required Forms before equipment pickup and/or participation

Forms may be submitted in person or emailed to [email protected].

• Medical Treatment, Consent, and Information Form
  
• Player/Parent Handbook & SIGNED Code of Conduct
• Waiver and Release of Liability 
• Proof of Identity
  • For any new players: an ORIGINAL proof of identity must be shown AND a COPY to leave behind to keep on record. Acceptable Forms of Proof of Identity (NEED ONE):
    • Birth Certificate 
    •  Passport 
    •  State ID (age 10 or older)
