Avon Grove Wildcats Forms

Welcome to the Wildcats Forms Area. These forms are provided for convenience and may be printed, filled out and returned to the Wildcats in lieu of obtaining these forms directly from Wildcats staff.  The text of these forms may not be altered from their original content in any way.  

These forms require signatures and therefore cannot be submitted online.  You may complete them online, print them out, sign them and return directly to an Avon Grove Wildcats Staff Member or mail to:

All Football and Cheerleading Registrations will now be completed through the website.  You will need to complete the online form when registrations are open.

Below you will find a description of special areas of the online registration process:

Football Player Registration - (Registration is currently open until June 30th)
Use the online registration form to signup and pay for all Football Players. Keep in mind that an Original Birth Certificate and a Copy of the Birth Certificate will be required for age verification when the players get their photos taken for their Bert Bell League ID Cards. 

Cheerleader Registration - (Registration is currently open until June 30th)
Use the online registration form to signup and pay for all Cheerleaders. Keep in mind that an Original Birth Certificate and a Copy of the Birth Certificate will be required for age verification when the players get their photos taken for their Bert Bell League ID Cards.

Player Contract
This form is required for agreement and compliance with the by-laws and guidelines of the Avon Grove Wildcats Organization.

Sports Physical Examination
Fill out and sign the parent section and have your doctor fill out and sign the physician section no more than 6 months prior to the beginning of football camp (after 2/1/10).  No equipment will be distributed without the submission of a completed Sports Physical Exam.  Therefore, your child will not be permitted to practice until the completed documentation is handed in.  

Code of Conduct
All players, parents, coaches and staff members must sign this Code of Conduct and abide by its terms and conditions.  A copy will be kept on file with organization.  Please read the Code of Conduct Disciplinary Actions Statement.
